How it works

Bupropion is a selective inhibitor of the neuronal re-uptake of catecholamines (noradrenaline and dopamine) with minimal effect on the re-uptake of indolamines (serotonin) and does not inhibit either monoamine oxidase. The mechanism of action of bupropion as an antidepressant is unknown. However, it is presumed that this action is mediated by noradrenergic and/or dopaminergic mechanisms.

Usage And Safety

Dosage

Adults : Initially 150mg daily for 6 days , increase to 150mg twice daily allowing minimum 8 hr between doses. Max single dose, 150mg; max daily dose, 300mg. Start treatment while still smoking. A target date to stop smoking should be set within the first 2 wk and treatment should be continued for 7-9wk. Discontinue if no effect is seen by 7 wk. Elderly : 150mg daily. Children : Not Recommended. Or As directed by your physician.

Side Effects

Agitation, dry mouth, insomnia, headache/migraine, nausea/vomiting, constipation, and tremor, taste disorders, dizziness, concentration disturbances.

Drug Interactions

Inhibitors/inducers of the CYP2B6 isoenzyme (e.g., orphenadrine, thiotepa, cyclophosphamide, ticlopidine, and clopidogrel) , paroxetine, sertraline, norfluoxetine, and fluvoxamine as well as nelfinavir and efavirenz , antidepressants (SSRIs, many tricyclics), beta-blockers, antiarrhythmics, and antipsychotics , nortriptyline, imipramine, desipramine, antipsychotics e.g., haloperidol, risperidone, thioridazine , beta-blockers (e.g., metoprolol), antiarrhythmics e.g., propafenone, flecainide , MAO Inhibitors , Levodopa and Amantadine , antipsychotics, other antidepressants, theophylline, systemic steroids, Alcohol.

Indication

It is indicated for the treatment of major depressive episodes.

When not to Use

It is contraindicated in patients with hypersensitivity to bupropion or any of the excipients , In patients with a current seizure disorder or any history of seizures , In patients with a known central nervous system tumour.

Precautions

Precaution

All patients being treated with antidepressants for any indication should be monitored appropriately and observed closely for clinical worsening, suicidality, and unusual changes in behavior, especially during the initial few months of a course of drug therapy, or at times of dose changes, either increases or decreases.

Warnings

Warning 1

The recommended dose of modified release bupropion should not be exceeded, since bupropion is associated with a dose-related risk of seizure.

Warning 2

It should be discontinued promptly if patients experience hypersensitivity reactions during treatment.

Warning 3

There is limited clinical experience of the use of bupropion to treat depression in patients with cardiovascular disease. Care should be exercised if it is used in these patients. However, bupropion was generally well tolerated in studies for smoking cessation in patients with ischaemic cardiovascular disease.
